Community Service Tracking Form
WCHS BETA Community Service Form
BETA members will submit a record of their service hours for all community service projects. In addition to submitting this form, BETA members are required to keep documentation of their service hours on the form provided. This form will be turned in at the end of each semester for documentation.
Forms are located in the bottom of the magazine rack outside of the counselor's office.
